Appointment

The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

   
Payment Plan - We can establish a short term (three to six month) in office payment plan for your convenience. Please call for details.  Any balance over 90 days is charged an interest of 1.5%.

Dental Insurance

Understanding insurance benefits is often difficult.  Please contact our office if you have questions regarding your coverage and benefits, we are happy to help you.   We are preferred providers for Delta Dental Premier, AlwaysCare/Unum, Beam and many Cigna plans. We will file claims with  other traditional insurance plans. If we are not currently in your network, we can still file all claims for you, and you will likely have benefits at our office. Any of our front desk staff can research the level of your benefits on any type of insurance.

We are happy to file your insurance claims for you, please confirm your coverage is in effect prior to your appointment.   You are responsible for any balance remaining after your insurance company makes their payment.  

Dr. Hays or Dr. Folk will be happy to outline treatment options for you based on your optimal dental health and cosmetic concerns.
